Brockport students will need to complete a home health screening each morning before getting on the bus or arriving to school. The Qualtrics pre-screening tool will be sent to parents' email and secondary students' school email daily. A temperature check is required to attest your child does not have a temperature of over 100 degrees. If your child is not cleared, you will see a red x and will need to keep your student home for the day. You will receive a call from the school to check in and provide further direction. Sending a student to school who has exhibited COVID-19 symptoms in the last 24 hours places other students and staff members at risk of becoming ill. It also increases the likelihood that schools will have to temporarily close. When in doubt, keep your child home and consult your personal physician.
QUALTRICS SELF-SCREENING QUESTIONNAIRE:
1. Have you traveled internationally or from a state with widespread community transmission of COVID-19 per the NYS Travel Advisory in the past 14 days?
2. Have you tested positive for COVID-19 in the past 14 days?
3. Have you knowingly been in contact with someone who has tested positive for COVID-19 in the last 14 days?
4. Have you experienced any of these symptoms in the past 24 hours (not due to an underlying condition)?
Temperature greater than 100 degrees

Sore throat (not due to allergies)
Muscle or body aches Loss of smell or taste
Nasal congestion
Nausea or vomiting
