Registration & Records

    The Registration Office is responsible for:

    Student Registration UPK - 12:

    • Universal Pre-Kindergarten (UPK) students must be 4 years of age prior to December 1st of the year they enroll. 
    • Kindergarten students must be 5 years of age prior to December 1st of the year they enroll.
      • Please complete and return the "Household Information form" below to ensure you receive the Kindergarten and UPK mailings. Information regarding Kindergarten is usually sent in March, and UPK information is sent in April.

    Information on Rights of Parents from the Family Education Rights and Privacy Act (FERPA) An educational agency or institution shall give full rights under the Act to either parent, unless the agency or institution has been provided with evidence that there is a court order, State statute, or legally binding document relating to such matters as divorce, separation, or custody that specifically revokes these rights. (Authority: 20 U.S.C. 1232g)

    Student Database Management:

    Notify the Office of Registration & Records if you have a change in address, telephone number, email address, contact changes, custody, etc – please select the link below "Student Information Change Form"  
     

    District Records Management:

    Student Records and Transcripts. If you would like to obtain an official/unofficial copy of your high school transcript, or any other school records, please select the link below “Request for Information”
     

    Student Attendance and Enrollment Verification:

    Medical/Immunizations:

    Complete the “Request for Information" form below.
     
    Due to HIPAA Regulations, medical records cannot be released without the authorization of the patient or the patient's parent/guardian. For enrollment eligibility purposes, it may be necessary for the District to request immunization records, health records or other medical information.
