Any student who is in learning remotely in Cohort 4 who would like to update and change your instructional model from REMOTE to HYBRID, please complete this form (found at the link below) by Friday, October 16, 2020.
A new form for each student requesting a change in model must be submitted. It is important to note, families selecting hybrid will be placed in a cohort based on current numbers.
In addition, not all requests may be fulfilled based on space and programming needs. The next hybrid instructional model will begin on Monday, November 9, 2020.
